This form is to file a complaint and/or notice of a possible covenant violation. Actual covenant violations are determined by the Board of Directors of CKHA and/or the Architectural Control Committee and may require a legal decision.

Please be advised that any possible violation that can not be resolved by the BOD, the ACC or the homeowner may end up in a court of law, with a mediator or arbitrator.

This notice may become part of the permanent records of the association and subject to legal authorities. At which time you may be required to appear in court as a witness to help resolve any disputes.
